Peraturan Presiden 13/2026 (Peraturan Presiden Nomor 13 Tahun 2026 tentang Pengelolaan Kesehatan) is an Indonesian presidential regulation on health management, the governance of health efforts and health resources by the central government, provincial, regency and city governments and village governments. It was signed by President Prabowo Subianto and promulgated on 11 March 2026, in Lembaran Negara 2026 No. 25, and took effect on that date.[1][2] It was issued to implement provisions of the health law, Undang-Undang Nomor 17 Tahun 2023, and it revoked Peraturan Presiden Nomor 72 Tahun 2012 on the national health system.[3][4][2:1] It is one of the framework instruments of health sector regulation in Indonesia.
The regulation places health management within a national health system run in tiers at central and regional level, and organises it in four stages: policy-setting, planning and budgeting, implementation, and monitoring and evaluation. It binds regional and village policy to central policy, and empowers the central government to reprimand and apply disincentives to regional and village governments whose planning or reporting departs from national plans.[5][6][7] A lengthy annex allocates responsibilities for each health effort and each health resource among ministries, agencies and village governments, and an inter-ministerial coordination forum is established under the coordinating minister for human development.[8][9][10] CRPG's regulatory tracker published a summary of the regulation in July 2026.[11]
The preamble states that the regulation implements Pasal 21 ayat (3) and Pasal 416 of Undang-Undang Nomor 17 Tahun 2023 on health, and its only other legal basis is Pasal 4 ayat (1) of the Constitution, on the President's executive power.[3:1][2:2] It replaced Peraturan Presiden Nomor 72 Tahun 2012 on the national health system (Sistem Kesehatan Nasional), which Pasal 28 revokes.[4:1]
Pasal 1 defines health management (Pengelolaan Kesehatan) as the application of governance to health efforts and health resources by the central, provincial, regency or city and village governments, in an integrated and mutually supporting way, to secure the highest attainable level of public health. Health efforts (Upaya Kesehatan) are activities that maintain and improve public health in promotive, preventive, curative, rehabilitative or palliative form, and health resources (Sumber Daya Kesehatan) are everything needed to deliver them.[12] Under Pasal 2, the regulation aims to achieve the highest level of public health through individual and public health efforts and to improve coordination in delivering effective, efficient, quality and affordable health services.[12:1]
Pasal 3 provides that health management takes place within a national health system, defined as an integrated and dynamic arrangement of health efforts and health resources, carried out in tiers at the centre and in the regions and with public participation. Pasal 4 lists the outputs expected of it (access, coverage, equity, safety and quality, and affordability of services) and its intended results, including responsiveness, efficiency and resilience of the health system and financial protection for the whole population.[5:1]
Pasal 6 lists twenty-four health efforts, from maternal, infant, child, adolescent, adult and elderly health to nutrition, mental health, the control of communicable and non-communicable disease, environmental health, disaster health, blood services, organ transplantation, the safety of pharmaceuticals, medical devices, food and addictive substances, forensic medical services and traditional health services; delivered as services, they run through primary and advanced health care. Pasal 7 lists the health resources: health facilities, health human resources, health supplies, health information systems, health technology, health financing and other resources set by the Minister of Health. Under Pasal 8, health management also covers extraordinary events and epidemics.[5:2]
Pasal 9 sets the four stages of health management. Policy is set by the central, provincial and regency or city governments, in legislation or in other forms; the central government's authority is exercised by ministries and non-ministerial agencies within their functions and in coordination. Regional governments must follow central policy when setting regional health policy, and village governments manage the health matters within village authority by reference to both central and regional policy.[6:1]
For planning, Pasal 14 requires the central government to set priority policies and performance indicators through the long-term and medium-term national development plans, a health master plan (rencana induk bidang Kesehatan) and the annual government work plan. The medium-term plan and the health master plan are the reference for the five-year and annual health plans of ministries, agencies and regional governments, and regional and village plans must also follow the master plan.[13] Implementation, under Pasal 18, consists of each level of government discharging its responsibilities and of inter-agency coordination and synchronisation.[13:1]
All levels of government monitor and evaluate the management of health efforts and resources. Pasal 20 empowers the central government to issue oral or written reprimands and disincentives to provincial, regency, city or village governments where monitoring shows that their health planning conflicts with national plans, that their health efforts and resources do not follow the national development strategy, or that they have failed to report on health targets or on health budget spending.[7:1]
Pasal 21 makes every level of government responsible for health management within its authority and for encouraging public participation. The responsibilities of regional governments follow the general legislation, and they are additionally responsible for supplying the resources needed for priority health programmes. Those of the central government, broken down by ministry and agency, and of village governments are set out in the annex.[8:1] Under Pasal 22, the central government strengthens affirmative policies for remote, disadvantaged, border and island regions, based on a mapping of regional fiscal capacity and health priorities.[8:2]
The annex is organised in six parts: health efforts (twenty-three subjects, from maternal health to traditional medicine), primary health care, advanced health care, health resources (facilities, human resources, supplies, information systems, technology and financing), extraordinary events and epidemics, and community participation. For each subject it lists the duties of the Ministry of Health and of other ministries and agencies. For maternal health, for example, the Ministry of Health sets service standards, delivers services, manages resources and monitors care before pregnancy, in pregnancy, at delivery and after delivery; the Ministry of Home Affairs directs regional governments to align their policies, staffing, facilities and budgets; and the Ministry of Manpower sets policy on the rights of working mothers.[9:1] Under the financing heading the Ministry of Health prepares the health master plan and the benefits and tariff policy of the national health insurance programme.[9:2]
Pasal 23 to Pasal 27 require coordination and synchronisation of health policy among ministries, agencies and other parties, to prevent and resolve policy problems, consolidate implementation and strengthen the health system. An inter-ministerial health coordination forum is formed with a steering tier chaired by the coordinating minister for human development and culture and an executive tier chaired by the Minister of Health; its detail is left to the Minister.[10:1]
The division of responsibility for health efforts among regional governments is reflected in the guideline for provincial and district health offices issued in the same period, Permenkes 2/2026, which took effect on the same day, 11 March 2026. The management of extraordinary events and epidemics named in Pasal 8 is regulated in detail by Permenkes 1/2026.[5:3][14][15] The annex also assigns duties to the food and drug authority, whose instruments include Peraturan BPOM 10/2026 on nutrition labelling.[9:3][16]
