Peraturan Menteri Kesehatan 14/2026 (Peraturan Menteri Kesehatan Nomor 14 Tahun 2026 tentang Perubahan Kedua atas Peraturan Menteri Kesehatan Nomor 30 Tahun 2020 tentang Organisasi dan Tata Kerja Unit Pelayanan Kesehatan Kementerian Kesehatan) is an Indonesian ministerial regulation that amends, for the second time, the organisational charter of the Ministry of Health's health service units (Unit Pelayanan Kesehatan, UPK). It was signed by the Minister of Health on 17 June 2026 and took effect on its promulgation on 19 June 2026.^[1][2] It belongs to the body of health sector regulation in Indonesia and to the reorganisation of ministry technical units described at public administration in Indonesia.

The amendment places the units under the directorate general responsible for advanced health management, removes the general administration subdivision from their structure so that each unit consists of its head and holders of functional and executor positions, and requires the head of each unit to apply the government performance accountability system, national development risk management and national digital transformation.^[3][4] Existing posts and officials remain in place until new positions are formed and filled.^[5] CRPG's regulatory tracker published a summary of the regulation in July 2026.^[6]

¶ Background

Under Peraturan Menteri Kesehatan Nomor 30 Tahun 2020, a UPK is a technical implementation unit whose task is to provide health services to the officials and employees of the Ministry of Health and to the public, and which sits under the Director General responsible for health services. As enacted in 2020, its structure consisted of a general administration subdivision (subbagian administrasi umum) and a group of functional positions, with the subdivision head holding a supervisory or echelon IV.a post.^[7] The 2020 charter was first amended by Peraturan Menteri Kesehatan Nomor 13 Tahun 2023.^[8]

The preamble of the 2026 amendment gives three grounds: the optimisation of performance and the simplification of bureaucracy, the fact that several provisions of the charter as amended no longer matched legal developments and organisational needs, and the approval of the reorganisation by the Minister for State Apparatus Empowerment and Bureaucratic Reform. It was issued to implement Pasal 240 of Peraturan Menteri Kesehatan Nomor 21 Tahun 2024 on the organisation of the Ministry of Health, as amended by Peraturan Menteri Kesehatan Nomor 17 Tahun 2025. Its legal bases include Undang-Undang Nomor 39 Tahun 2008 on state ministries as amended by Undang-Undang 61/2024, the health law Undang-Undang Nomor 17 Tahun 2023 and its implementing Peraturan Pemerintah Nomor 28 Tahun 2024, and Peraturan Presiden Nomor 161 Tahun 2024 on the Ministry of Health.^[8:1]

¶ Provisions

The amending article, Pasal I, contains fifteen numbered changes to the charter. The new Pasal 1 keeps the definition of a technical implementation unit (Unit Pelaksana Teknis, UPT) as an autonomous work unit carrying out specific technical operational or technical support tasks of its parent organisation, and redefines the Director General and the Directorate General by reference to advanced health management (pengelolaan kesehatan lanjutan) in place of health services.^[3:1][7:1]

¶ Staffing

The new Pasal 6 reduces the structure of a UPK to its head and the holders of functional and executor positions (jabatan fungsional dan jabatan pelaksana); Pasal 7, which had set the tasks of the general administration subdivision, is deleted, and Bab VI is renamed accordingly. Under the new Pasal 10, functional positions provide functional services according to expertise and skill, executor positions carry out public services and government and development administration, and both are grouped by job family. Pasal 11 provides that the number of functional and executor groups is set by need on the basis of job analysis and workload analysis. Pasal 12 allows position holders and groups to be assigned individually or in work teams, with assignments set by the performance appraisal official or the head of the organisational unit.^[3:2][4:1]

¶ Accountability, coordination and leadership

Pasal 13 requires the head of a UPK to apply the government agency performance accountability system, national development risk management and national digital transformation. Pasal 14 requires the work of the units within a UPK to rest on business processes applying coordination, integration, synchronisation and collaboration, and provides that those business processes are set by the Minister. Pasal 17 extends the same principles to relations within the ministry, with other government agencies and with other institutions, supported by data and information interoperability, and Pasal 19 makes each unit leader responsible for directing and coordinating staff.^[4:2][9] Under the amended Pasal 22 and Pasal 23, the head of a UPK holds an administrator or echelon III.a post and is appointed and dismissed by the Minister; the second paragraph of Pasal 22, which had graded the subdivision head, is deleted. Pasal 27 of the 2020 charter, a transitional provision for officials appointed under the 2012 predecessor charter, is deleted, and the organisation chart in the annex is replaced.^[9:1][7:2]

¶ Transition

Pasal II provides that all existing positions and the officials holding them under the charter remain in place and continue to perform their duties until new positions are formed and new officials appointed under the amendment, and that the amendment takes effect on the date of its promulgation.^[5:1] The regulation is one of several amendments to the charters of Ministry of Health technical units issued in June 2026, among them Permenkes 9/2026 on the environmental health laboratories and Permenkes 10/2026 on the health equipment and facility safety units.^[10]
