Peraturan Menteri Kesehatan 2/2026 (Peraturan Menteri Kesehatan Nomor 2 Tahun 2026 tentang Pedoman Nomenklatur Perangkat Daerah dan Unit Kerja pada Perangkat Daerah yang Menyelenggarakan Urusan Pemerintahan Bidang Kesehatan) is an Indonesian ministerial regulation that sets the guideline for the names, structure, tasks and functions of the provincial and district or city health offices (Dinas Kesehatan) and their internal units. It was signed by the Minister of Health on 18 February 2026 and took effect on its promulgation on 11 March 2026, replacing Peraturan Menteri Kesehatan Nomor 36 Tahun 2023 on the same subject.[1][2][3] It belongs to the body of health sector regulation in Indonesia and to the rules on the organisation of regional government described at public administration in Indonesia.
The regulation classifies health offices into three types by the number of divisions they may have, and for each type prescribes the divisions and their functions. It allows health affairs to be merged with up to two related government affairs in one office where a region does not qualify for a separate health office, provides for regional technical units, regional hospitals and community health centres, and favours work teams of functional and executor staff over structural sub-units.[4][5][6][7] Regions were given one year from entry into force to adjust the nomenclature of their offices.[8] CRPG's regulatory tracker published a summary of the regulation in August 2026.[9]
Provincial, regency and city health offices are regional apparatus (perangkat daerah) that carry out the health affairs falling within regional authority.[10] The preamble of Permenkes 2/2026 states that alignment between central and regional government in health affairs is needed to support national health development goals and the success of health transformation, and that the issue of Peraturan Menteri Kesehatan Nomor 21 Tahun 2024 on the organisation of the Ministry of Health required the organisation of provincial and district or city health offices to be adjusted. It found that Permenkes 36/2023, the previous guideline, no longer matched legal developments and organisational needs.[11]
Its legal bases include Undang-Undang Nomor 23 Tahun 2014 on regional government, the health law Undang-Undang Nomor 17 Tahun 2023 and its implementing Peraturan Pemerintah Nomor 28 Tahun 2024, Undang-Undang Nomor 20 Tahun 2023 on the civil service, Peraturan Pemerintah Nomor 18 Tahun 2016 on regional apparatus as amended in 2019, and a 2022 regulation of the Minister for State Apparatus Empowerment and Bureaucratic Reform on working systems for the simplification of bureaucracy.[11:1] Pasal 2 provides that the guideline is the basis for drawing up the organisation and working procedures of provincial and district or city health offices.[10:1]
Pasal 1 defines a health office as the implementing element for health affairs that fall within regional authority, and a regional technical implementation unit (Unit Pelaksana Teknis Daerah, UPTD) as an organisation carrying out technical operational or technical support activities of the office. Under Pasal 4 to Pasal 6, the office is named Dinas Kesehatan Provinsi in a province and Dinas Kesehatan Kabupaten/Kota in a regency or city, and answers to the governor, regent or mayor through the regional secretary. Pasal 7 gives the offices the task of assisting the regional head in health affairs and in assistance tasks (tugas pembantuan), with functions of formulating and implementing operational policy, technical guidance and supervision, and evaluation and reporting across primary and community health, disease control, advanced health care, pharmacy, medical devices, health financing and health human resources.[10:2]
Pasal 8 divides health offices into three types, each with one secretariat: Type A with at most four divisions (bidang), Type B with at most three and Type C with at most two. The typology itself is set under other legislation.[4:1] A Type A office consists of the secretariat and divisions for primary and community health, for disease control, for advanced health care, pharmacy and medical devices, and for health resources.[12] A Type B office combines advanced health care, pharmacy, medical devices and health resources in one division, and a Type C office also combines primary and community health with disease control.[13][14]
The secretariat coordinates tasks and provides administrative support across the office. Its listed functions include planning and budgeting, financial and asset reporting, procurement, the synchronisation of the plans and budgets of regional hospitals and units run as regional public service agencies (badan layanan umum daerah), legal drafting, personnel administration, public information and complaints, health data and information systems, minimum service standards, and the government internal control system including risk management. A provincial secretariat also synchronises the planning of the district and city health offices within the province.[12:1]
The division for primary and community health covers family health, vulnerable groups, health promotion and the governance, facilities and quality of primary care, including care in remote, border, island and forest areas and for isolated customary communities. The disease control division covers non-communicable and communicable disease, immunisation, surveillance and health quarantine, and environmental health, including early warning of and response to extraordinary events, epidemics and health crises.[12:2] The division for advanced health care, pharmacy and medical devices covers the licensing of advanced health facilities and clinical services, telemedicine and electronic medical records, the referral system, the governance of regional and teaching hospitals, accreditation and patient safety, the certification of production and distribution facilities for pharmaceuticals and medical devices, supply chains and the safety of health equipment and facilities; at provincial level it also facilitates the settlement of medical disputes under the national health insurance programme. The health resources division covers the planning, provision, deployment, quality and welfare of medical and health personnel, health reserve personnel and cadres, regional health accounts, the costing of minimum service standards, and the verification and payment of national health insurance contributions funded from regional budgets; at provincial level it also facilitates the formation of provincial panels that examine complaints of professional disciplinary violations.[12:3]
The secretariat and each division may consist of sub-units (subbagian or seksi), groups of functional and executor positions, or both; tasks are allocated so that all work is covered, sub-unit names follow proportional workload, and the groups may take the form of work teams.[12:4]
Where a region does not meet the requirements for a separate health office, Pasal 25 allows health affairs to be merged in one office with affairs of similar character or close linkage, namely social affairs, women's empowerment and child protection, population control and family planning, population administration and civil registration, and community and village empowerment, up to a total of three affairs. A merged office may rise one type or gain one division, in the combinations set out in Pasal 25 ayat (6); Pasal 26 sets a fallback structure for merged offices that do not follow those combinations.[5:1]
Under Pasal 27, a health office may form UPTDs for work such as public health laboratories, health services, the safety of health equipment and facilities, health supplies, integrated emergency services and health services in health crises. Pasal 28 to Pasal 30 allow provincial offices to form regional hospitals and district or city offices to form regional hospitals and community health centres (puskesmas). A regional hospital is a special organisational unit with autonomy in finance, regional assets and personnel, led by a director who reports to the head of the office; a community health centre is a functional unit whose head answers to the head of the district or city office.[6:1]
Pasal 31 allows a health office to adjust its working mechanism by forming work teams in the secretariat and divisions, composed of functional and executor staff and appointed by the head of the office. Pasal 32 requires the office's work to rest on business processes applying coordination, integration, synchronisation and collaboration, and Pasal 33 requires job analyses, job maps, workload analyses and task descriptions for every position.[7:1]
Pasal 34 keeps all existing positions and officials under the 2023 guideline in place until new positions are set and officials appointed, and Pasal 35 requires the adjustment of health office nomenclature within one year of entry into force, that is by 11 March 2027. Pasal 36 revokes Permenkes 36/2023, and Pasal 37 brings the regulation into force on promulgation.[8:1][3:1][2:1] The guideline's allocation of epidemic and health crisis response to the disease control division sits alongside Permenkes 1/2026 on extraordinary events, epidemics and health crises, and the division of health management between levels of government was set in the same period by Perpres 13/2026.[15][16]
